assistive technology referral for assessment
can be made following the
district guidelines. This is a step-by-step
process to ensure the comprehensive assimilation
of important information. The documents,
presentations, and forms on this page provide
the resources necessary for taking the
appropriate steps for considering assistive
technology implementation. The type of
assessment you need will determine which
of these three referral forms to use.

6513 is used when the M-Team is requesting
an assistive technology
assessment.

by the
school as
part of the assistive technology
implementation strategy.If
a referral is requested by
the M-Team, copies of form
FM#7068 and/or FM# 7067 (depending
on documentation
need)
is sent with FM# 6513, FM#
4958 (Informed Notice of Reevaluation
Review Meeting
and/or Consent for Reevaluation)
and a copy of
the current IEP.
